Toxic anterior segment syndrome (TASS)

Definition: Acute post surgical inflammation (non-infectious) due to toxic substance in the anterior chamber.

Clinical features:

  1. Usually present 1-2 days after surgery with corneal oedema (limbus to limbus), hypopyon, fibrin and raised IOP.

  2. No vitritis and respond well to topical steroids.

  3. Check for any retained lens material.

Monitor closely as high risk of corneal decompensation, glaucoma or cystoid macular oedema.

HOT TIP: When unsure if it is TASS, treat as endophthalmitis.
